Download a DA form to lodge with the Department

The Department deals with applications where the Minister for Planning is the consent authority under Part 4 of the Environmental Planning and Assessment Act 1979. There are two types of forms provided:

  • forms that you can use to apply for approval under the Act
  • forms that you can use to give us notice as required under the Act.

Before you download and complete a form please contact the Department to determine whether we have any special information requests. We can also advise you which form/s you will need to complete. Some tips on using the forms are provided below.

 

A word of warning: Each of the forms has been prepared in good faith to assist applicants. Unfortunately, the Department cannot warrant that the rtf forms will be downloaded by different electronic systems in the format in which they were originally designed and that they will be free from error.

1. Development approval (DA required)  
Development application and supplement

This form is required to apply for Part 4 development consent from the Minister for Planning under section 78A of the Environmental Planning and Assessment Act 1979.

Application for construction certificate
Use this form to apply for a construction certificate to carry out building work or subdivision work before starting work

2. Starting work  
Notice to commence building or subdivision work and appointment of a principal certifying authority
Used this form to tell us and the local council that you have appointed a principle certifying authority. Also to give the local council at least 2 days notice before you start work

3. Work ends  
Compliance certificate
Used this form to certify that the conditions of your consent have been satisfied, that work has been properly carried out, or that a plan or design complies with a standard

4. To occupy or change the use of a building  
Application for an occupation certificate
Used this form to apply for permission to occupy or use a new building or change the use of an existing building

Fire safety certificate
Use this form if you are the owner of a building, or the owner’s agent. You will need to submit this certificate with an application for an occupation certificate. A copy of the certificate also needs to be given to the Commissioner of New South Fire Brigades and displayed in the building in a prominent position

Fire safety notice
Use this form if you are to occupy part of a building next to an exit that is required to be enclosed in fire-resisting construction. You will need to display a fire safety notice next to that exit

5. Building certificate

Use this form to apply for a building certificate over part or all of a building as per section 149B of the Environmental Planning and Assessment Act 1979

6. To register a subdivision  
Application for a subdivision certificate
Use this form to apply for a subdivision certificate. As a landowner, you will need the certificate to register your plan of subdivision with Land and Property Information New South Wales

Some tips on using the forms

Each form is provided in rich text format (rtf) and portable document format (pdf).

 

The rtf version will allow you to complete a form on-screen. rtf files are compatable with both Microsoft Word and Word Perfect applications. Each rtf form have been 'frozen' so that you cannot accidentally delete any information or any relevant section provided on the form.

 

To complete an rtf version of the form on-screen:

  1. Use the tab button to move between the check boxes and the white sections that must be completed
  2. Click on a check box with the left button on your mouse to cross it
  3. Type information directly into each relevant white section on the form - the box will expand to fit the content.

To keep the page layout as you intend when the rtf version of your completed form is printed:

  1. Check the position of page breaks
  2. Before you print a page make sure you select A4 paper and Portrait orientation
  3. Adjust your margin settings (file/page set-up in MS Word)

The pdf version can be used to print a form and complete it by hand. Please note that to view and print the pdf files you need to install Adobe Acrobat Reader on your computer.
