The Department deals with applications where the Minister for Planning is the consent authority under Part 4 of the Environmental Planning and Assessment Act 1979. There are two types of forms provided:
- forms that you can use to apply for approval under the Act
- forms that you can use to give us notice as required under the Act.
Before you download and complete a form please contact the Department to determine whether we have any special information requests. We can also advise you which form/s you will need to complete. Some tips on using the forms are provided below.
A word of warning: Each of the forms has been prepared in good faith to assist applicants. Unfortunately, the Department cannot warrant that the rtf forms will be downloaded by different electronic systems in the format in which they were originally designed and that they will be free from error.

The table below lists the forms, and the relevant clause in the Environmental Planning and Assessment Regulation 2000 that sets out the information each form needs to include.
Some tips on using the forms
Each form is provided in rich text format (rtf) and portable document format (pdf).
The rtf version will allow you to complete a form on-screen. rtf files are compatable with both Microsoft Word and Word Perfect applications. Each rtf form have been 'frozen' so that you cannot accidentally delete any information or any relevant section provided on the form.
To complete an rtf version of the form on-screen:
- Use the tab button to move between the check boxes and the white sections that must be completed
- Click on a check box with the left button on your mouse to cross it
- Type information directly into each relevant white section on the form - the box will expand to fit the content.
To keep the page layout as you intend when the rtf version of your completed form is printed:
- Check the position of page breaks
- Before you print a page make sure you select A4 paper and Portrait orientation
- Adjust your margin settings (file/page set-up in MS Word)
The pdf version can be used to print a form and complete it by hand. Please note that to view and print the pdf files you need to install Adobe Acrobat Reader on your computer.
